Section I
This Agreement executed on the date shown below by and between House of Dragons herein after referred to as the Center, and the buyer, who has caused his/her signature to be affixed hereto, hereinafter referred to as Buyer.
The Center agrees to provide instruction in Taekwondo to the Enrollee during the scheduled instruction hours of the Center. Enrollee understands that it is his or her responsibility to attend instruction sessions. Enrollee understands that the entire course fee stated above is due and payable according to the terms of this agreement whether or not Enrollee chooses to attend instructional sessions provided by the Center. In consideration for the Center’s promises to instruct and provide instruction facilities, Enrollee agrees and promises to pay the course fees shown above.
Enrollee agrees to abide by the rules and regulations governing the conduct and operation of instruction sessions, students, and observers, in order to promote efficiency, safety, and recognition of the instruction certification, as prescribed by the Center. Enrollee acknowledges that rules and regulations governing the Center have been adopted, and may be changed from time to time, and Enrollee agrees to abide by all such rules and regulations as adopted or hereafter amended. The hours of operation may be changed at the discretion of the Center.
The Center reserves the right to revoke or terminate any membership. Enrollee agrees that all exercises and/or courses are undertaken at members own risk. The Enrollee understands the procedures and exercises involved in instruction and participation as explained to him/her by an assistant of the Center. The Enrollee understands that there is a risk of personal injury involved in the course of instruction and with this knowledge agrees to indemnify and save harmless the Center from all losses caused by accident or injury to the Enrollee, or to third persons, who may be Enrollee of Center, in the event that either the Enrollee or said third person is injured in any way during the performance and execution of exercises. The company owning the Center and the agents and employees shall not be responsible for damaged, lost or stolen articles, inside or outside the facility. The Center reserves the right to amend or add to these rules and conditions and to adopt new rules and conditions as it may deem necessary.
If an Enrollee terminate his/her membership, no refunds will be given, all discounts are forfeit, and the balance of what has been paid versus the cost of full tuition for the months attended is immediately due. Make up classes are not given for classes missed by participants unless it is scheduled ahead of time.
I, the Enrollee or the Member, irrevocably authorize Center, its successors and assigns, and those acting under its authority, to copy, use, publish, for art, advertising, trade, or any other lawful purpose whatsoever, photographic portraits, pictures or videotapes of me, in which I may be included in whole or in part.
In consideration for the training obtained, I agree that I will not, during the continuance of this agreement, and for a period of three (3) years following termination of my association with Center, engage in teaching Taekwondo or offering similar training or instruction within a five (5) mile radius of any school operated by Center without the written authorization of same.

Section II
I understand that prices are not negotiable
I understand that House of Dragons’ (HOD) membership months are 4 weeks long. This is due to HOD being closed 4 weeks each year (Spring Break, 4thof July week, and the 2 week FISD Winter Break). HOD does not charge for the weeks it is closed.
I understand that my Consumer Credit Agreement (the CCA) is a month-to-month, 3 month, 6 month, or 12 month recurring auto draft, meaning that my financial institution account/credit card will be debited every month on the bill date until a written 30-day Cancellation Notice Request Form is submitted.
I understand that as protection for my financial institution account/credit card, I must submit a 30-day written Cancellation Notice in order to terminate or downgrade my CCA or Membership.
I understand that if I want to upgrade or downgrade my CCA, I need to give at least 48 hour notice to complete the request so it will reflect the correct changes on my auto-draft billing cycle.
I understand that HOD can freeze memberships, but not billing, for a maximum of 60 days in any 12 month period.
- Requires 1 week advanced notice in writing.
- Must be in 30 Day increments; i.e. either one 60 Day Freeze or two 30 Day Freezes in a 12 month period.
- Billing will continue and the memberships end date will be extended out the requisite number of weeks, prepaid.
I understand that missed classes can be made up, provided your membership is current and in good standing. However missed classes are not owed by HOD for any reason.
- Memberships, however, will not be extended due to absences outside of the control of HOD.
- Any missed classes are forfeited upon terminating my membership
I understand that Memberships that auto-draft from a debit/credit card incur a 5% service charge.
I understand that refunds will not be given for any reason; including but not limited to any delays, postponements, or cancellations due to public health reasons.
I understand that, should I enroll in a 3, 6, or 12 month membership and later terminate my Membership early for any reason, all payments on tuition are forfeited and I understand that no refunds will be given. The remaining weeks on the pre-paid membership can be transferred to a family member or friend who is not a current House of Dragons student.
For safety reasons, and, in order to ensure that each client receives a proper warm-up period, students will not be allowed to enter a class 10 minutes after the class has started.
I understand that students entering our studio will be in contact with children, families, and employees who are also at risk of community exposure. I understand that no list of restrictions, guidelines, or practices will 100% remove the risk of exposure to COVID-19, as the virus can be transmitted by persons who are asymptomatic and before some people show signs of infection. I understand that I play a crucial role in keeping everyone in the facility safe and reducing the risk of exposure by following the CDC, state, and local recommendations.
